Health & Safety Compliance Questionnaire

Does the company have a health and safety policy, which includes a general statement, defined responsibilities and general arrangements to help the organisation manage its operational risks. Is it signed?

Is the policy supplemented by written arrangements to control hazards in your workplace?

Does the company have a current employer’s liability insurance policy to the value of £5m or above?

Have you appointed a competent person/s as defined in the Management of Health and Safety at Work Regulations and has that person received formal training? i.e. IOSH Managing Safely as a minimum?

Is the "Health and Safety Law, what you should know" poster on display in the company and have the boxes for the additional information been completed?

Do new employees receive induction training from a checklist, specific to their role, is it signed off by the employee and their line manager?

Is the employer or the person who has control over the premises aware of their responsibilities for fire safety under the Regulatory Reform (Fire Safe) Order 2005 and have they put the appropriate arrangements in place?

Do you have a fire evacuation procedure for employees, contractors and people visiting the premises and has it been communicated to each of these categories of people?

Are arrangements in place to check on a regular basis that fire exits (both inside and out) free of obstruction and serviceable at all times?

Do you have suitable and sufficient risk assessments documented that include Machinery, COSHH, Manual Handling, Fire, Noise and Transport. And have the control measures been put in place to reduce the risk to the lowest practicable means?

Has a formal system for inspecting and testing guards, interlocks and emergency stop buttons been introduced on a regular basis, and is this documented?

Are forklift truck drivers trained by a qualified instructor?

Are accidents entered in the accident book, dangerous occurrences and near misses promptly investigated and the results recorded and is the company aware of the requirements of the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations and does it comply with them?

Do you advise contractors of your health & safety policy, and give them details of any significant hazards, in particular asbestos, which may affect them and the way they work?

When applicable, are arrangements in place, for undertaking risk assessments for young employees including work experience and trainees?
